Vacating your Bed

On your day of discharge please be ready to vacate your bed by 11am and arrange to be collected by 12 mid-day. This is necessary to prepare for other patients being admitted to hospital.
 

Follow-up Appointment?

If you need a follow-up appointment, this will be discussed with you prior to your discharge.
 
Your follow-up appointment will be given to you on discharge by ward staff or sent out to you in the post.
 
If you do not receive an expected follow-up appointment in the post, please contact the ward that you were discharged from.
 

Discharge Letter

Hospital staff will prepare a discharge letter.  This is a summary of medical information about your treatment in hospital and ongoing services that have been arranged for you. This letter will be sent to your General Practitioner.
 

GP Appointment?

You may wish to make an appointment to see your General Practitioner following discharge from the hospital.
 

Public Health Nurse

If applicable your local Public Health Nurse will have been informed about your discharge and this will have been discussed with you prior to your discharge.
 

Personal Belongings

Prior to leaving the hospital, ensure that you have all your personal belongings and that any valuables or medicines being kept for safe keeping are returned to you.
 

Discharge Prescription

Prior to leaving the hospital, ensure that you have your discharge prescription and that you understand your medication schedule for when you go home.
